Open	April 14th(Fri), 2017 7:30 ～ 18:00 15th(Sat), 2017 7:30 ～ 18:00 16th(Sun), 2017 7:30 ～ 16:00	Please inform us in advance, if you change time of your use.
Capacity	Approximately 10／each day
Eligibility	From 3 months old to preschool

Fee	Charge JPY5,000(tax included)/1 day per a child ※Please do not cancel and change time of your use without emergency. ※Please contact us surely if you reluctantly change time of your use.
Reservation	Please make a reservation by E-mail, including the information bellow. We will reply by e-mail to confirm your reservation with an application form attached. Insurance	The entrusting babysitter company joins the insurance and covers in case of any accidents. JRC 2017, the 76th Annual Meeting of the Japan Radiological Society, the 73rd Annual Scientific Congress of the JSRT, the 113th Scientific Meeting of the Japan Society of Medical Physics do not take any responsibility.

JRC 2017 Nursery terms of service

Eligibility	Children of JRC 2017 participant and members
Open	April 14th(Fri), 2017 7:30 ～ 18:00 15th(Sat), 2017 7:30 ～ 18:00 16th(Sun), 2017 7:30 ～ 16:00	Please inform us in advance, if you change time of your use.
Fee	Charge JPY5,000(tax included)/1 day per a child ※Please do not cancel and change time of your use without an emergency. ※Please contact us surely if you reluctantly change time of your use.
Belongings	① The identification card of parents (a health insurance card / a driver's license / passport ,etc.) ② Application form (Please hand it to the childcare staff on your first day.） ③ Name card of Congress participation. ④ Lunch(when day care time takes at 12:00),Snack (when day care time takes at 15:00), child’s formulas, baby bottle, a drink (barley tea is available at Kid’s Square), diapers, change of clothes, towel, several pieces of plastic bags (for dirty clothes cases).
Meal	① We do not provide any meals at our nursery. Please pick up your child during lunch time or bring child’s meal so that we can assist eating. ② We serve only what you bring for meal, drink and snacks.
For parents	① Please take your child to the restroom before leaving him or her at nursery. ② Please be reminded we can not accept your child when he or she has fever over 37.5　℃ , infectious diseases, is in poor physical condition, or is not being suitable for group nursery. ③ In case of an emergency, the kid’s room staff will contact you through mobile phone. Please inform the kid’s room staff the emergency contact number by all means. Please take and pick-up your child(ren) punctually. ④ The kid’s room staff cannot give any medication. ⑤ Please take and pick-up your child(ren) punctually. ⑥ Please put the names on all belongings.
Change and cancellation	Please contact by April 7th (Fri) 2017 as below
Insurance	① We join the insurance in case of any accidents / injury.
